Lenalidomide + dexamethasone is a combination therapy primarily used to treat multiple myeloma, a cancer of plasma cells. Lenalidomide is an immunomodulatory agent and thalidomide derivative that works by altering cytokine production, regulating T cell co-stimulation, enhancing natural killer (NK) cell-mediated cytotoxicity, and directly inhibiting the cullin ring E3 ubiquitin ligase complex via binding to cereblon. This leads to the degradation of key transcription factors (IKZF1 and IKZF3), which are essential for malignant plasma cell survival[4][6]. Dexamethasone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressive properties; in this combination it enhances the anti-myeloma effect of lenalidomide[5]. The primary indication for this combination is multiple myeloma, both as initial therapy and as maintenance following autologous stem cell transplant[1][2][8].
