Overview

Lenalidomide + dexamethasone + donor lymphocyte infusion (DLI) is a combination regimen primarily used in the treatment of multiple myeloma and other hematologic malignancies. Lenalidomide is an immunomodulatory agent that works by enhancing immune function, inhibiting angiogenesis, and inducing apoptosis of malignant cells. Dexamethasone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressive properties; it also induces apoptosis in certain cancer cells and enhances the efficacy of lenalidomide. Donor lymphocyte infusion (DLI) involves the administration of lymphocytes from a stem cell transplant donor to boost graft-versus-tumor effects after allogeneic hematopoietic stem cell transplantation. This combination aims to maximize anti-myeloma activity through direct cytotoxicity (lenalidomide/dexamethasone) and immune-mediated mechanisms (DLI). Lenalidomide was developed by Celgene (now part of Bristol Myers Squibb), while dexamethasone is an older generic drug produced by various manufacturers.
