Overview

Lenalidomide + gemcitabine is an investigational combination therapy consisting of two small molecule drugs with distinct but potentially complementary mechanisms of action. Lenalidomide is an immunomodulatory agent and thalidomide analogue with anti-angiogenic, antiproliferative, and pro-apoptotic properties. It modulates the immune system by enhancing T cell and natural killer (NK) cell function, inhibiting pro-inflammatory cytokines, and suppressing angiogenesis. Gemcitabine is a nucleoside analog used as a chemotherapeutic agent; it inhibits DNA synthesis by incorporating into DNA during replication, leading to apoptosis in rapidly dividing cells. The combination has been studied primarily in advanced pancreatic cancer and relapsed or refractory peripheral T-cell lymphomas to assess feasibility, safety, and potential synergistic antitumor effects[1][3][4]. Preclinical data suggest that lenalidomide may sensitize tumor cells to gemcitabine by downregulating the MAPK/ERK signaling pathway associated with chemoresistance[5].
