Lenalidomide + melphalan is a combination regimen used primarily in the treatment of multiple myeloma and related plasma cell disorders. Lenalidomide is an immunomodulatory agent that acts by altering the substrate specificity of the Cereblon E3 ubiquitin ligase complex (CRL4^CRBN), leading to degradation of key transcription factors (IKZF1, IKZF3) essential for malignant plasma cell survival and stimulating immune responses via increased interleukin 2 and interferon gamma production[6]. Melphalan is an alkylating agent that cross-links DNA strands at the N7 position of guanine, disrupting DNA synthesis and transcription, resulting in cytotoxicity to both resting and dividing tumor cells[5][1]. The combination has been studied for use in multiple myeloma as well as AL amyloidosis[8].
