Overview

Lenvatinib + PD-1 monoclonal antibody is a combination therapy regimen primarily utilized in the treatment of advanced hepatocellular carcinoma (HCC). Lenvatinib is an oral multi-kinase inhibitor that targets vascular endothelial growth factor receptors (VEGFR1-3), fibroblast growth factor receptors (FGFR1-4), platelet-derived growth factor receptor alpha (PDGFRα), KIT, and RET, thereby exerting potent anti-angiogenic and anti-proliferative effects. The PD-1 monoclonal antibody component (which in clinical practice often includes agents such as camrelizumab, sintilimab, or tislelizumab) is an immune checkpoint inhibitor that blocks the programmed cell death protein 1 (PD-1) receptor on T cells. This blockade prevents interaction with its ligands (PD-L1 and PD-L2), restoring the anti-tumor activity of cytotoxic T lymphocytes. The combination is designed to synergistically enhance therapeutic efficacy by modulating the tumor microenvironment and overcoming immune evasion. The First Hospital of China Medical University has specifically investigated this regimen, often in conjunction with hepatic arterial infusion chemotherapy (HAIC), for patients with high tumor burden HCC and portal vein tumor thrombus (PVTT).
