Lepetegravir + lenacapavir pacfosacil is an investigational, once-weekly oral combination regimen developed by Gilead Sciences for the treatment of HIV-1 infection. The regimen combines lepetegravir (GS-1720), a long-acting integrase strand transfer inhibitor (INSTI), with lenacapavir pacfosacil (GS-1614), a prodrug of the first-in-class capsid inhibitor lenacapavir. Lepetegravir works by blocking the HIV-1 integrase enzyme, thereby preventing the integration of viral DNA into the host cell genome. Lenacapavir, the active moiety of lenacapavir pacfosacil, disrupts the HIV-1 capsid at multiple stages of the viral lifecycle, including nuclear transport, virus assembly, and capsid core formation. This combination is currently being evaluated in the Phase 2/3 WONDERS1 study as a maintenance therapy for virologically suppressed individuals with HIV-1, aiming to provide a potent, long-acting oral alternative to daily antiretroviral therapy.
