Lerapolturev + anti-PD-1 checkpoint inhibitor is a combination regimen being evaluated primarily for the treatment of anti-PD-1–refractory metastatic melanoma. **Lerapolturev** is a novel investigational oncolytic immunotherapy derived from a genetically modified Sabin type 1 polio virus (formerly known as PVSRIPO), which selectively targets the poliovirus receptor (CD155) broadly expressed on solid tumors. It activates innate and adaptive immunity through antigen presenting cell engagement, direct tumor cell lysis, and recall of polio vaccine-specific immunity. Anti-PD-1 agents are monoclonal antibodies that block the programmed death receptor-1 (PD-1) pathway to enhance T cell-mediated anti-tumor responses[3]. The combination aims to synergistically boost antitumor immune activity, particularly in tumors resistant to prior anti-PD-1 monotherapy[1][3].
