Letrozole + inositol is an investigational combination regimen being evaluated for the induction of ovulation in women with polycystic ovary syndrome (PCOS), particularly those who have shown resistance to letrozole monotherapy. Letrozole is a non-steroidal aromatase inhibitor that works by suppressing estrogen biosynthesis, which reduces negative feedback on the hypothalamus and pituitary gland, leading to increased secretion of follicle-stimulating hormone (FSH) and subsequent follicular development. Inositol, typically administered as myo-inositol or D-chiro-inositol, serves as an insulin-sensitizing agent. It improves the metabolic profile by enhancing insulin-stimulated glucose uptake and reducing hyperinsulinemia, which helps to lower ovarian androgen production and restore regular ovulatory cycles. The combination is currently being studied in Phase 2 clinical trials to improve pregnancy rates and ovulatory outcomes in the PCOS population.
