leucovorin + 5-fluorouracil + oxaliplatin + targeted therapy is a combination chemo-immunotherapy regimen used as a first-line treatment for metastatic colorectal cancer. It consists of the FOLFOX chemotherapy backbone (oxaliplatin, 5-fluorouracil, and leucovorin) administered alongside a targeted monoclonal antibody, typically either bevacizumab (targeting VEGF) or cetuximab (targeting EGFR). Oxaliplatin acts as a DNA alkylating agent, 5-fluorouracil is an antimetabolite that inhibits thymidylate synthase, and leucovorin is a folate analogue that enhances the inhibitory effect of 5-fluorouracil. The targeted therapy component provides additional efficacy by either inhibiting angiogenesis (bevacizumab) or blocking growth factor signaling (cetuximab). This regimen is currently being evaluated as the standard-of-care comparator in Phase 3 clinical trials conducted by Shanghai Zhongshan Hospital to assess the efficacy of newer irinotecan liposome-based combinations.
