Leucovorin + fluorouracil + bevacizumab (FOLF+B) is a combination chemotherapy and anti-angiogenic regimen used as a backbone treatment for various advanced solid tumors, particularly colorectal cancer. The regimen consists of fluorouracil (5-FU), a pyrimidine analog antimetabolite that inhibits thymidylate synthase to disrupt DNA and RNA synthesis; leucovorin (folinic acid), a biochemical modulator that enhances the binding of 5-FU to its target enzyme, thereby increasing its cytotoxic activity; and bevacizumab, a recombinant humanized monoclonal antibody that targets vascular endothelial growth factor A (VEGF-A) to inhibit tumor-associated angiogenesis. In clinical research, FOLF+B is frequently employed as a standard-of-care comparator or combination partner, as seen in Phase 1 trials evaluating novel agents like the topoisomerase I inhibitor PEEL-224.
