This combination therapy is an investigational chemo-immunotherapy regimen being evaluated for the first-line treatment of metastatic colorectal cancer (mCRC). It consists of the standard FOLFOX chemotherapy backbone (oxaliplatin, leucovorin, and fluorouracil), the EGFR-targeting monoclonal antibody cetuximab, and QL1706. QL1706 is a novel bifunctional antibody (MabPair) that simultaneously targets programmed cell death protein 1 (PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). The regimen is specifically being studied in patients with RAS/BRAF wild-type, left-sided primary tumors, where EGFR inhibition is typically more effective. The combination aims to leverage the cytotoxic effects of chemotherapy, the targeted inhibition of EGFR signaling, and the dual checkpoint blockade of PD-1 and CTLA-4 to enhance anti-tumor immune responses and improve clinical outcomes.
