Levetiracetam + melatonin is a combination of two agents, levetiracetam and melatonin. Levetiracetam is an anticonvulsant medication primarily used as adjunctive therapy for the treatment of partial onset seizures, myoclonic seizures, and primary generalized tonic-clonic seizures in patients with epilepsy. Its mechanism of action involves binding to synaptic vesicle protein 2A (SV2A), which modulates neurotransmitter release and reduces neuronal excitability. Melatonin is a hormone produced by the pineal gland that regulates circadian rhythms and sleep-wake cycles; it acts mainly through MT1 and MT2 receptors in the suprachiasmatic nucleus (SCN) to promote sleep. Melatonin supplements are commonly used for insomnia, circadian rhythm disorders, jet lag, and as adjunctive therapy for sleep disturbances in various neurological conditions including epilepsy[2][3][6]. The combination may be considered when both seizure control (levetiracetam) and management of sleep disturbances or circadian rhythm regulation (melatonin) are desired; however, co-administration can increase central nervous system depression risk[1][2][4][6].
