Chirocaine Heavy is an extemporaneously prepared hyperbaric (heavy) local anesthetic solution used for spinal anesthesia, particularly in women undergoing elective caesarean delivery. It is a combination of levobupivacaine (the S-enantiomer of bupivacaine), fentanyl (a potent opioid analgesic), and dextrose, which increases the solution's baricity (density) relative to cerebrospinal fluid. Levobupivacaine acts by blocking voltage-gated sodium channels in nerve membranes to inhibit nerve conduction, while fentanyl acts as a mu-opioid receptor agonist to provide synergistic analgesia. This specific formulation is not commercially licensed and was evaluated in a Phase 4 clinical trial sponsored by Balikesir University to compare the efficacy of different solution densities.
