It works by inhibiting voltage-gated sodium channels in nerve membranes, which prevents the influx of sodium ions and the subsequent depolarization required for nerve impulse conduction. In the context of the Cairo University trial, this combination (19 ml of 0.5% levobupivacaine and 1 ml of 0.9% normal saline) is administered via ultrasound-guided spermatic cord block to manage postoperative pain in male patients undergoing testicular sperm extraction surgery. The saline component serves as a volume-matching control for the experimental adjuvant, neostigmine.
