Overview

Levodopa + dopa decarboxylase inhibitor (LD + DDCI) is a combination therapy used primarily for the treatment of Parkinson's disease and related movement disorders. Levodopa (L-DOPA) is a prodrug that crosses the blood-brain barrier and is converted to dopamine in the brain, compensating for dopamine deficiency characteristic of Parkinson's disease. However, when administered alone, much of the levodopa is metabolized peripherally before reaching the brain. To prevent this peripheral conversion and reduce side effects such as nausea and cardiovascular complications, it is co-administered with a dopa decarboxylase inhibitor (DDCI), such as carbidopa or benserazide. The DDCI does not cross the blood-brain barrier but inhibits peripheral metabolism of levodopa, increasing its availability to the central nervous system and improving efficacy while reducing systemic side effects[3][4]. This combination remains a gold standard for symptomatic control in Parkinson’s disease[1][3][4].
