Levodopa + standard care refers to the use of levodopa, a dopamine precursor, in combination with the usual non-experimental therapies for Parkinson's disease (PD). Levodopa is converted to dopamine in the brain and is considered the most effective pharmacologic treatment for alleviating motor symptoms such as bradykinesia and rigidity in PD. Standard care typically includes supportive measures like physical therapy, occupational therapy, speech therapy, and may include other medications as clinically indicated. Levodopa is often administered with a peripheral decarboxylase inhibitor (such as carbidopa) to increase its bioavailability in the brain and reduce side effects. The mechanism of action involves replenishing depleted dopamine levels in the striatum by providing a substrate that crosses the blood-brain barrier and is then converted into active dopamine[6][8][10]. This combination remains first-line therapy for symptomatic management of Parkinson's disease.
