Levodopa + tetrabenazine is a combination of two small molecule drugs with opposing effects on the dopaminergic system. Levodopa is a dopamine precursor that increases dopamine levels in the brain and is primarily used to treat Parkinson's disease by improving motor symptoms such as rigidity, tremor, and bradykinesia. Tetrabenazine is a vesicular monoamine transporter 2 (VMAT2) inhibitor that depletes presynaptic dopamine stores, reducing hyperkinetic movements such as chorea and dyskinesias. This combination has been explored experimentally for managing levodopa-induced dyskinesias in advanced Parkinson’s disease patients, where tetrabenazine may reduce involuntary movements caused by chronic levodopa therapy without significantly worsening parkinsonian motor features[2][3]. However, this approach remains investigational due to potential risks of exacerbating parkinsonism and limited clinical trial data[1][2][3].
