Overview

A 7-day investigational triple therapy regimen, abbreviated as LAcR, developed by Buddhist Tzu Chi General Hospital for the eradication of Helicobacter pylori infection. The regimen consists of three primary components: levofloxacin (a fluoroquinolone antibiotic), amoxicillin/clavulanate (a combination of a penicillin-class antibiotic and a beta-lactamase inhibitor), and rabeprazole (a proton pump inhibitor). This combination was specifically designed and evaluated in Eastern Taiwan to overcome high local resistance rates to standard clarithromycin- and metronidazole-based therapies. Levofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, preventing DNA replication. Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, while clavulanate protects amoxicillin from degradation by bacterial beta-lactamases. Rabeprazole increases gastric pH, which stabilizes the antibiotics and enhances their activity against H. pylori.
