levofloxacin + budesonide is a combination therapy consisting of a fluoroquinolone antibiotic and a synthetic corticosteroid, currently being investigated by Assiut University for the treatment of acute exacerbations in mechanically ventilated patients with non-cystic fibrosis bronchiectasis. Levofloxacin is a broad-spectrum antibacterial agent that inhibits the bacterial enzymes DNA gyrase and topoisomerase IV, which are essential for DNA replication, transcription, and repair. Budesonide is a potent glucocorticoid that exerts anti-inflammatory effects by binding to the glucocorticoid receptor, thereby reducing the production of inflammatory cytokines and mediators in the airways. In the context of this clinical application, the combination is administered via bronchoscopic local instillation to deliver high concentrations of both agents directly to the pulmonary site of infection and inflammation, aiming to improve outcomes such as ventilator liberation and mortality.
