Levofloxacin + cefepime is a combination of two antibiotics used together for the treatment of severe bacterial infections, particularly in cases where monotherapy may be insufficient or when dealing with multidrug-resistant organisms. Levofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, leading to inhibition of DNA replication and cell death. Cefepime is a fourth-generation cephalosporin (a beta-lactam antibiotic) that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, resulting in cell lysis and death. This combination has been reported as effective in treating severe respiratory infections caused by pathogens such as Aeromonas caviae, especially after failure of single-agent therapy[1][10]. The rationale for combining these agents includes broadening the antimicrobial spectrum, enhancing bactericidal activity through potential synergy, and reducing the risk of resistance development[8].
