ZENALEV O is a fixed-dose combination pharmaceutical product developed by Opes Health Care, containing the fluoroquinolone antibiotic levofloxacin and the nitroimidazole antiprotozoal agent ornidazole. Levofloxacin, a third-generation fluoroquinolone, acts by inhibiting the bacterial enzymes DNA gyrase (topoisomerase II) and topoisomerase IV, which are essential for DNA replication, transcription, and repair, thereby exerting a bactericidal effect against a wide range of Gram-positive and Gram-negative bacteria. Ornidazole is a nitroimidazole derivative that undergoes reductive activation within anaerobic microorganisms and protozoa, forming reactive intermediates that cause DNA strand breakage and inhibition of DNA synthesis. This combination is primarily indicated for the treatment of mixed infections, particularly gastrointestinal infections such as diarrhea and dysentery of mixed bacterial and protozoal origin, as well as pelvic inflammatory disease and other polymicrobial infections where both aerobic and anaerobic pathogens are suspected.
