Overview

This is a combination formulation used primarily in imaging procedures such as MR arthrography. It contains three active components: - **Lidocaine** is an amide-type local anesthetic that stabilizes neuronal membranes by inhibiting ionic fluxes required for the initiation and conduction of nerve impulses, resulting in local anesthesia. - **Adrenaline (epinephrine)** is a vasoconstrictor that prolongs the effect of lidocaine by reducing its systemic absorption, thereby maintaining higher tissue concentrations at the site of administration. - **Gadolinium** refers to a gadolinium-based contrast agent (GBCA), most commonly as gadopentetate dimeglumine or similar chelated forms, which enhances MRI images by altering magnetic properties within tissues. This mixture is typically prepared immediately before use for intra-articular injection during MR arthrography to provide both anesthesia and enhanced imaging contrast. The addition of adrenaline/epinephrine helps reduce bleeding and prolongs anesthetic action. Gadolinium provides high-quality MRI visualization without significant risk of free gadolinium ion release when mixed with lidocaine and adrenaline[1][2][4]. The combination has been shown to be safe for confirming intra-articular placement prior to MRI scans.
