DR SSB (Distal Radius Single Shot Block) is a trial-specific regional anesthetic combination developed by NYU Langone Health for the management of postoperative pain following distal radius fracture fixation. The intervention consists of a single-shot peripheral nerve block using a mixture of 2% lidocaine with 1:200,000 epinephrine and 0.5% bupivacaine with 1:300,000 epinephrine. Lidocaine provides a rapid onset of anesthesia, while bupivacaine offers a prolonged duration of analgesic effect. Epinephrine is included as a vasoconstrictor to delay systemic absorption, thereby extending the duration of the block and reducing potential systemic toxicity. This combination is administered perineurally to block voltage-gated sodium channels, preventing the transmission of pain signals from the surgical site. It has been evaluated in Phase 4 clinical trials (NCT01843309) to compare its efficacy against continuous infusion techniques, such as the On-Q pump, in controlling immediate postoperative pain and reducing rebound pain and opioid consumption.
