lidocaine + dexamethasone is a combination therapy being evaluated by Boston Children's Hospital for the management of post-traumatic headaches (PTH) and associated neck pain. The treatment involves the administration of these agents via Occipital Nerve Block (ONB) or Cervical Medial Branch Block (CMBB). Lidocaine acts as a local anesthetic by blocking voltage-gated sodium channels, providing rapid but temporary pain relief by inhibiting nerve signal transmission. Dexamethasone, a potent synthetic glucocorticoid, is included to reduce inflammation of the trigeminovascular and cervical plexus networks, potentially offering more prolonged therapeutic benefits. This approach targets the underlying stressors, such as high cervical facet joint inflammation and traumatic cranial neuralgias, which are thought to contribute to the persistence of PTH after traumatic brain injury.
