Overview

Lidocaine + ketamine is a combination of two pharmaceutical agents commonly used in pain management and anesthesia. Lidocaine is an amide local anesthetic that works primarily by blocking sodium channels, thereby inhibiting nerve signal transmission and providing analgesia. Ketamine is a dissociative anesthetic that acts as an antagonist at the N-methyl-D-aspartate receptor (NMDA receptor), reducing central sensitization and modulating pain perception. Ketamine also interacts with opioid receptors, sodium channels, potassium channels, and may affect neurotransmitter release such as glutamate. The combination leverages both peripheral (lidocaine) and central (ketamine) mechanisms to provide multimodal analgesia for acute or chronic pain conditions—including neuropathic pain—and can be administered intravenously or transdermally[1][2][4][6]. This combination has been studied for use in perioperative anesthesia, pediatric sedation, chronic low back pain, neuropathic pain syndromes, and other neurologic or chronic pain disorders[1][2][4][6].
