Overview

This is a combination drug consisting of lidocaine, vitamin B12 (commonly as cyanocobalamin or methylcobalamin), and betamethasone. Lidocaine is a local anesthetic that works by blocking sodium channels, thereby inhibiting nerve signal transmission and providing pain relief. Vitamin B12 is an essential water-soluble vitamin involved in DNA synthesis, myelin formation, and cellular metabolism; it has been studied for its potential analgesic effects in neuropathic pain conditions. Betamethasone is a potent synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressive properties; it acts by binding to the glucocorticoid receptor to modulate gene expression and suppress pro-inflammatory mediators. This combination has been used via local injection (intra-articular or subcutaneous) for the management of various pain syndromes such as acute herpetic neuralgia and knee osteoarthritis. Clinical studies suggest that this combination can reduce inflammatory markers (e.g., IL-1, MMP-3), increase TIMP-1 levels, relieve pain intensity, improve quality of life scores, and potentially lower the incidence of postherpetic neuralgia when administered early after symptom onset[2][3]. The rationale behind combining these agents lies in their complementary mechanisms—local anesthesia from lidocaine for immediate relief; anti-inflammatory action from betamethasone; neurotrophic support from vitamin B12.
