Overview

Lidocaine Hydrochloride Injection, without epinephrine (marketed under the brand name Xylocaine) is an amide-type local anesthetic formulation containing lidocaine hydrochloride as the active ingredient. It acts primarily by blocking voltage-gated sodium channels, which inhibits nerve conduction and pain signaling. When administered systemically as a continuous intravenous infusion, it also exhibits potent anti-inflammatory properties, including the inhibition of the arachidonic acid cascade and suppression of eicosanoid and prostaglandin production. This formulation is specifically prepared without epinephrine, distinguishing it from combination local anesthetics that include a vasoconstrictor. It is widely approved for local and regional anesthesia, and has been investigated by institutions such as the University of Alabama at Birmingham in clinical trials (e.g., NCT01043211) for the management of postoperative pain following adult spine surgery.
