Overview

Linagliptin + basal insulin is a combination therapy used in the management of type 2 diabetes mellitus, particularly for patients whose blood glucose levels are inadequately controlled with basal insulin alone. Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone levels (GLP-1 and GIP), thereby enhancing glucose-dependent insulin secretion and suppressing glucagon release. Basal insulins are long-acting insulins that provide steady background insulin coverage to control fasting plasma glucose. The combination improves glycemic control without increasing the risk of hypoglycemia or weight gain compared to placebo, making it especially useful in elderly patients or those at higher risk for hypoglycemia[1][2][10]. Linagliptin is primarily developed by Boehringer Ingelheim and Eli Lilly, while various companies manufacture different formulations of basal insulins.
