Overview

**Linagliptin + warfarin** refers to the concomitant administration of linagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor developed by Boehringer Ingelheim primarily for type 2 diabetes, and warfarin, a vitamin K antagonist anticoagulant widely used for prevention and treatment of thromboembolic disorders. Linagliptin acts as an oral antihyperglycemic by inhibiting DPP-4, thereby increasing incretin levels (GLP-1 and GIP), which in turn increases insulin secretion and decreases glucagon secretion. Warfarin inhibits the synthesis of vitamin K-dependent clotting factors (II, VII, IX, X). The combination does not have a unique brand or trade name, nor is it a co-formulated product, but may be administered together in clinical practice. There are no well-documented pharmacokinetic or pharmacodynamic interactions between linagliptin and warfarin at this time; however, multiple studies highlight the extensive risk of drug interactions involving warfarin and the need for monitoring INR when warfarin is combined with any new medication, including antidiabetic agents[4][5].
