Linezolid + vancomycin is a combination of two antibiotics used primarily for the treatment of serious infections caused by Gram-positive bacteria, including multidrug-resistant strains such as methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE). Linezolid is an oxazolidinone antibiotic that inhibits bacterial protein synthesis by binding to the 23S portion of the 50S ribosomal subunit, thereby blocking the initiation complex formation required for translation. Vancomycin is a glycopeptide antibiotic that inhibits cell wall synthesis in susceptible bacteria by binding to D-Ala-D-Ala termini of cell wall precursor units. The combination has been studied for potential synergistic effects against resistant organisms; however, evidence suggests variable outcomes depending on pathogen and infection site. Some studies report additive or indifferent activity in vitro with occasional antagonism observed in vivo, particularly against MRSA endocarditis[1][2][5][7]. For VRE infections with high-level aminoglycoside resistance, some synergy has been reported without observed antagonism[2]. Clinical use should be guided by susceptibility patterns and infection characteristics.
