Linvoseltamab + nirogacestat is an investigational combination therapy for relapsed/refractory multiple myeloma, consisting of **linvoseltamab**, a bispecific monoclonal antibody targeting B-cell maturation antigen (BCMA), and **nirogacestat**, a small-molecule gamma-secretase inhibitor. Linvoseltamab activates cytotoxic T cells against BCMA-expressing myeloma cells, inducing cell death. Nirogacestat inhibits gamma-secretase, which prevents shedding of the BCMA antigen from myeloma cells, thereby maintaining expression of BCMA on the cell surface and potentially increasing sensitivity to BCMA-targeting therapies. The combination aims to enhance antitumor activity by sustaining the target for linvoseltamab. Linvoseltamab is developed and manufactured by Regeneron; nirogacestat is manufactured by SpringWorks Therapeutics. The combination is currently being investigated in clinical trials for multiple myeloma[1][7][2][4].
