This combination chemotherapy and targeted therapy regimen is being evaluated as a second-line treatment for advanced biliary tract cancer. It consists of liposomal irinotecan (a nanoliposomal formulation of the topoisomerase I inhibitor irinotecan), 5-fluorouracil (an antimetabolite), leucovorin (a folate analog that enhances 5-FU activity), and lenvatinib (a multi-kinase inhibitor). The regimen aims to inhibit tumor cell replication through DNA damage and metabolic disruption while simultaneously targeting angiogenesis and tumor growth pathways by inhibiting VEGFR, FGFR, and other receptor tyrosine kinases. It is currently being studied in a Phase II clinical trial sponsored by the Cancer Institute and Hospital, Chinese Academy of Medical Sciences, specifically for patients who have progressed after first-line therapy.
