Liposomal topotecan + cisplatin is a combination chemotherapy regimen investigated for the treatment of locally advanced cervical cancer. The regimen consists of Brakiva (a liposomal formulation of the topoisomerase I inhibitor topotecan) and the platinum-based agent cisplatin, administered in conjunction with pelvic radiation therapy. Liposomal encapsulation of topotecan is intended to improve the drug's pharmacokinetic profile by increasing its half-life and tumor accumulation compared to the standard formulation. Cisplatin acts by forming DNA cross-links, which interfere with DNA replication and trigger apoptosis. This combination was evaluated by the Gynecologic Oncology Group (GOG) in a Phase I dose-escalation study (GOG-9929) to determine the maximum tolerated dose and safety profile when combined with standard radiotherapy in patients with cervical carcinoma without extra-pelvic metastasis.
