Overview
Liposome bupivacaine is an extended-release formulation of the amide-type local anesthetic bupivacaine, utilizing the DepoFoam multivesicular liposomal delivery system. This technology allows for the slow release of bupivacaine over an extended period, providing postoperative analgesia for up to 72 hours. Its mechanism of action involves the reversible blockade of voltage-gated sodium channels on nerve membranes, which prevents the initiation and conduction of nerve impulses, thereby producing local anesthesia. Developed by Pacira BioSciences and marketed under the brand name Exparel, it is indicated for single-dose infiltration in adults to produce postsurgical local analgesia and as an interscalene brachial plexus nerve block for postsurgical regional analgesia. Recent clinical investigations, such as those conducted at the First Affiliated Hospital of Xinxiang Medical College, explore its efficacy in thoracic paravertebral blocks for patients undergoing lung resections.
