Liquid metformin (Aspargo Labs) is a novel oral solution formulation of the biguanide antihyperglycemic agent metformin, currently in Phase 1 development by Aspargo Labs for the management of type 2 diabetes mellitus. Metformin's primary mechanism of action involves the activation of AMP-activated protein kinase (AMPK) and the inhibition of mitochondrial complex I, which collectively lead to decreased hepatic glucose production (gluconeogenesis), reduced intestinal glucose absorption, and enhanced peripheral glucose uptake and utilization. Aspargo Labs is specifically evaluating two concentrations (100 mg/mL and 250 mg/mL) to characterize their pharmacokinetic profile, relative bioavailability, and gastrointestinal tolerability compared to standard immediate-release tablets. This liquid formulation is intended to provide a more flexible dosing option and an alternative for patients who have difficulty swallowing solid oral dosage forms or require precise dose titration.
