Liraglutide + linagliptin is a combination of two antidiabetic agents used in the management of type 2 diabetes mellitus. Liraglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist, which increases insulin secretion and decreases glucagon secretion in response to glucose, as well as slows gastric emptying. Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases endogenous incretin levels (GLP-1 and GIP), thereby enhancing insulin release and suppressing glucagon secretion. Both drugs act via incretin pathways but through different mechanisms—liraglutide directly stimulates the GLP-1 receptor, while linagliptin prevents degradation of endogenous incretins by inhibiting DPP-4[2][4][5]. The combination has been studied but is not recommended due to minimal additive benefit and potential for increased side effects; no fixed-dose commercial product exists for this combination[6][7][8].
