Lisaftoclax + homoharringtonine + cytarabine + aclarubicin (LHAA) is an investigational combination chemotherapy regimen being evaluated for the treatment of newly diagnosed intermediate- and high-risk acute myeloid leukemia (AML). The regimen combines lisaftoclax (APG-2575), a novel orally bioavailable BCL-2 selective inhibitor, with the established HAA backbone consisting of homoharringtonine (a protein synthesis inhibitor), cytarabine (an antimetabolite), and aclarubicin (an anthracycline). Lisaftoclax promotes apoptosis in cancer cells by targeting the anti-apoptotic BCL-2 protein and may overcome resistance mediated by MCL-1. This combination is currently being studied in a multicenter, randomized phase II/III clinical trial in young adults to compare its efficacy against other lisaftoclax-based regimens.
