Overview

Lithium + tranylcypromine is a combination therapy involving two distinct small molecule drugs used primarily in the management of treatment-resistant major depressive disorder. Lithium is a mood stabilizer commonly used for bipolar disorder and as an augmenting agent in depression, while tranylcypromine is an irreversible monoamine oxidase inhibitor (MAOI) indicated for major depressive episodes. The combination has been studied in patients with refractory depression, where adding tranylcypromine to ongoing lithium therapy has shown significant clinical improvement. The mechanism of action involves potentiation of monoaminergic neurotransmission via MAO inhibition by tranylcypromine and modulation of serotonergic and other central pathways by lithium[1][6]. This regimen should be used with caution due to the risk of significant drug interactions and potential adverse effects.
