Overview

Lithium + venlafaxine is a combination therapy involving two distinct pharmaceutical agents used primarily in the management of major depressive disorder, particularly in cases resistant to monotherapy. Lithium is a mood stabilizer commonly used for bipolar disorder and as an augmenting agent in treatment-resistant depression. Its mechanism includes modulation of neurotransmitter systems, neuroprotective effects, and enhancement of serotonergic activity. Venlafaxine is a serotonin-norepinephrine reuptake inhibitor (SNRI) antidepressant that increases synaptic concentrations of serotonin and norepinephrine by inhibiting their reuptake into presynaptic neurons. The combination is often employed when patients do not respond adequately to antidepressant monotherapy; lithium augmentation can enhance the antidepressant effect but may increase the risk of serotonin syndrome due to synergistic serotonergic activity[1][2][6]. There are no clinically significant pharmacokinetic interactions between these drugs[3]. This combination should be monitored closely for adverse effects such as serotonin syndrome.
