Overview

LMWH + rivaroxaban is a combination therapy involving low molecular weight heparin (LMWH), a parenteral anticoagulant, and rivaroxaban, an oral direct factor Xa inhibitor. Both agents are used for the prevention and treatment of venous thromboembolism (VTE), including in patients with cancer-associated thrombosis. LMWH acts by potentiating antithrombin III to inhibit factor Xa and thrombin, while rivaroxaban directly inhibits factor Xa without the need for cofactors. This combination may be used sequentially or as bridging therapy during transitions between parenteral and oral anticoagulation[1][2][4]. The primary indication is VTE management; both drugs reduce clot formation but via different mechanisms.
