LN-145 Gen 3 is an investigational third-generation autologous tumor-infiltrating lymphocyte (TIL) cell therapy developed by Iovance Biotherapeutics. It is specifically designed for the treatment of non-small cell lung cancer (NSCLC) in patients who have progressed after chemotherapy and anti-PD-1 therapy. The Gen 3 manufacturing process represents a significant advancement over previous iterations by utilizing core biopsies as the source material instead of requiring surgical tumor resection, and it features a shortened 16-day production timeline. The therapy involves harvesting naturally occurring T cells from a patient's tumor, expanding them ex vivo to billions of cells, and reinfusing them into the patient following a non-myeloablative lymphodepletion regimen. This is typically followed by the administration of interleukin-2 (IL-2) to support the expansion and effector function of the infused TILs within the tumor microenvironment.
