LOAd703 + gemcitabine is an experimental combination therapy for advanced pancreatic ductal adenocarcinoma, consisting of **LOAd703**, an oncolytic adenovirus gene therapy encoding immunostimulatory transgenes CD40L (CD40 ligand) and 4-1BBL (4-1BB ligand), administered via intratumoral injection, together with **gemcitabine**, a standard cytotoxic chemotherapy given intravenously. **LOAd703** selectively infects and replicates in tumor cells, inducing tumor cell lysis and expressing CD40L and 4-1BBL to stimulate dendritic cell maturation and expand effector T and NK cells, thereby enhancing anti-tumor immunity. When combined with gemcitabine, this regimen aims to both directly kill tumor cells and activate immune-mediated tumor destruction. The combination has been studied with or without nab-paclitaxel in phase 1/2 clinical trials for advanced or metastatic pancreatic cancer, showing manageable safety and promising antitumor activity.
