Overview

The combination of local anesthetic and morphine is used in neuraxial anesthesia (spinal or epidural) to provide effective perioperative and postoperative pain management. Local anesthetics block the conduction of nerve impulses that relay pain signals, while morphine, an opioid analgesic, acts on the central nervous system to relieve pain. When combined, these medications provide complementary effects: - Local anesthetics (commonly bupivacaine) provide immediate anesthesia and analgesia - Morphine provides longer-lasting postoperative pain relief This combination is particularly beneficial for procedures such as cesarean sections, joint replacements, and spinal surgeries. According to the American Society of Anesthesiologists Task Force on Acute Pain Management, there is Category A1 evidence showing improved pain scores when neuraxial morphine is combined with local anesthetics compared to neuraxial morphine alone. The combination allows for segmental analgesia without substantial motor blockade, which is especially important for mothers after cesarean section to care for their babies. Dosing varies by procedure, with typical intrathecal morphine doses being 100-300 μg, with doses higher than 300 μg carrying increased risk of respiratory depression.
