Overview

A combination chemotherapy regimen comprised of **lomustine**, **bleomycin**, **vinblastine**, and **doxorubicin**, most commonly referred to as **B-CAVe**. This regimen has primarily been studied as salvage therapy for patients with advanced or relapsed **Hodgkin's lymphoma**, particularly those who have failed or relapsed after first-line therapy such as MOPP (mechlorethamine, vincristine, procarbazine, prednisone) or ABVD (doxorubicin, bleomycin, vinblastine, dacarbazine)[1][2][3]. - **Mechanism of action:** - **Lomustine** is an alkylating agent (nitrosourea class) that cross-links DNA and inhibits DNA/RNA synthesis, leading to cell death. - **Bleomycin** induces DNA strand breaks through generation of free radicals. - **Vinblastine** is a vinca alkaloid that inhibits microtubule formation, blocking cell division. - **Doxorubicin** is an anthracycline antibiotic that intercalates into DNA, inhibits topoisomerase II, and generates free radicals, causing cytotoxicity[1][2][3]. The intent of the combination is to use non-overlapping mechanisms and toxicity profiles for improved antitumor effect in refractory Hodgkin's lymphoma.
