**Lomustine + etoposide + cyclophosphamide (LEC)** is an oral or intravenous combination chemotherapy regimen consisting of three alkylating and cytotoxic agents: lomustine (a nitrosourea alkylating agent), etoposide (a topoisomerase II inhibitor), and cyclophosphamide (a nitrogen mustard alkylating agent). It is primarily used as a second- or third-line treatment for relapsed small cell lung cancer (SCLC) and as a conditioning regimen for autologous hematopoietic stem cell transplantation (ASCT) in relapsed Hodgkin's and non-Hodgkin's lymphomas, often as an alternative to carmustine-based regimens like CBV due to drug shortages. Clinical studies report response rates of approximately 38% in SCLC, median survival of 4.4 months, and favorable 2-year overall survival (85%) in lymphoma ASCT settings with acceptable toxicity, though hematological adverse events and rare treatment-related mortality occur.[1][2][3]
