A multi-agent, all-oral chemotherapy combination used investigationally and in clinical practice settings for certain lymphomas in people with AIDS and studied for relapsed small cell lung cancer. Mechanistically, it combines two alkylating agents that cross the blood-brain barrier (lomustine and procarbazine) with an oxazaphosphorine alkylator (cyclophosphamide) and a topoisomerase II inhibitor (etoposide), aiming for synergistic cytotoxicity and CNS penetration. Typical study regimens administered lomustine on day 1, etoposide on days 1–3, and cyclophosphamide plus procarbazine on days 22–26 of 6-week cycles, often with G-CSF support. Primary toxicities are hematologic (myelosuppression). Primary indications explored include AIDS-related non-Hodgkin’s lymphoma, AIDS-associated Hodgkin’s disease, and second- or third-line small cell lung cancer.
