The L-TBC regimen is an intensified myeloablative conditioning (MAC) protocol developed by the Hematology Medical Center at the Second Affiliated Hospital of Army Medical University (Xinqiao Hospital) for patients with T-cell acute lymphoblastic leukemia (T-ALL) undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). This combination therapy consists of four small molecule alkylating agents: lomustine (CCNU), thiotepa, busulfan, and cyclophosphamide. The regimen is specifically designed to address the high risk of relapse and frequent central nervous system (CNS) involvement characteristic of T-ALL. Lomustine and thiotepa are included for their superior ability to cross the blood-brain barrier, providing essential CNS prophylaxis and treatment, while busulfan and cyclophosphamide provide robust systemic anti-leukemic activity and marrow ablation. Clinical studies have demonstrated that this modified regimen can improve survival outcomes and reduce relapse rates in high-risk T-ALL populations compared to standard conditioning protocols.
