This is a combination regimen consisting of **lonafarnib** (an oral farnesyltransferase inhibitor), **paclitaxel** (a microtubule-stabilizing chemotherapeutic agent), and **carboplatin** (a platinum-based alkylating agent). Lonafarnib inhibits the enzyme farnesyltransferase, preventing the farnesylation of certain proteins, including Ras, thereby interfering with signaling pathways important for cancer cell growth. Paclitaxel binds to and stabilizes microtubules, inhibiting their disassembly and thus blocking cell division. Carboplatin causes DNA crosslinking and damage, resulting in apoptosis of rapidly dividing cells. This combination has been studied in clinical trials for advanced ovarian cancer and non-small cell lung cancer, though the addition of lonafarnib to the standard paclitaxel-carboplatin regimen has not shown improved outcomes in ovarian cancer[1][2][4][5][7][8].
