Overview

The combination of lonafarnib, paclitaxel, and trastuzumab is a therapeutic regimen that has been studied for the treatment of HER2-overexpressing breast cancer. This combination brings together three distinct medications with complementary mechanisms of action. ## Composition and Mechanism Lonafarnib is a farnesyltransferase inhibitor that blocks protein farnesylation, which is important for the function of certain proteins involved in cell growth and survival. Paclitaxel is a taxane chemotherapy agent that stabilizes microtubules, preventing cell division and leading to cell death. Trastuzumab is a humanized monoclonal antibody that targets the HER2 protein, which is overexpressed in certain breast cancers[1][2]. The rationale behind combining these agents is that lonafarnib may enhance the anti-tumor effects of paclitaxel and trastuzumab. Studies have shown that lonafarnib potentiates the growth inhibitory effects of paclitaxel and enhances paclitaxel-induced mitotic arrest and apoptosis[5]. ## Clinical Development A phase I trial was conducted to determine the maximum tolerated dose (MTD), dose-limiting toxicities, safety profile, and preliminary efficacy of this combination in patients with HER2-positive advanced breast cancer[6]. The recommended dose established from this trial was: - Lonafarnib: 250 mg/day (125 mg twice daily) continuously - Paclitaxel: 175 mg/m² as a 3-hour infusion every 3 weeks - Trastuzumab: 4 mg/kg loading dose followed by 2 mg/kg weekly[6] ## Efficacy and Safety The combination showed promising preliminary antitumor activity with an objective response rate of 58% in patients with HER2-positive advanced breast cancer[6]. Common adverse events observed with this regimen included: - From paclitaxel and trastuzumab (cycle 1): alopecia, myalgia, sensory neuropathy, fatigue, arthralgia, leukocytopenia, and neutropenia - Additional adverse events after adding lonafarnib (cycle 2 onwards): diarrhea, nausea, dyspepsia, vomiting, and allergy[6] The study concluded that lonafarnib can be safely combined and tolerated with full doses of paclitaxel and trastuzumab in HER2-positive advanced breast cancer patients[2][6]. It's worth noting that while this specific three-drug combination has been studied, the two-drug combination of trastuzumab and paclitaxel is more commonly used in clinical practice for HER2-positive breast cancer and has demonstrated significant efficacy in this patient population[7][9].
