The combination of **lopinavir**, **ritonavir**, **emtricitabine**, and **tenofovir disoproxil fumarate** is an oral antiretroviral regimen used in the treatment of HIV-1 infection. Each component has a distinct mechanism of action targeting different stages of the HIV replication cycle. Lopinavir is a protease inhibitor, administered with ritonavir, which strongly inhibits CYP3A-mediated metabolism, thereby boosting lopinavir plasma concentrations and efficacy. Emtricitabine and tenofovir disoproxil fumarate are nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs) that block the activity of the viral reverse transcriptase enzyme, preventing viral RNA conversion to DNA. This four-drug combination is not available as a fixed-dose co-formulation but is used as a potent multi-class regimen in clinical practice for either initial treatment or salvage therapy in antiretroviral-experienced patients, especially when a high barrier to resistance or potent viral suppression is required[1][3][7].
